• In order for the employer application to be valid, you must check the box on page 2 confirming that all the information on the form is correct.

To submit the form electronically:
When the form has been completed, click on the “Submit Form” button and follow the instructions that pop up. The form will be emailed to WorkSource.
You may save and/or print this form.

To submit the form manually:
Employer Application: when the form has been completed, print and sign at the bottom of page 2.
Schedule A: no signature necessary.
Fax the form to WorkSource: 250-479-8114.
